I) To obtain Tamilnadu Drug Licence
1) Pre-requisites for Premises Area
- Retail: 10 sq. m
- Wholesale: 10 sq. m
- Retail & Wholesale: 15 sq. m
- Storage facilities racks, refrigerator and A/C have to be provided.
2) Documents Required
- a) Form 19,20,21 as applicable
- b) Covering letter to the Assistant Director of Drugs Control of the concerned zone
- c) Authorization document signed by the authorized signatory
- d) Declaration form filled and signed by the applicant, pharmacist, or competent person
- e) Details of premises
- *ownership document
- *rental agreement
- *signed plan
- f) Pharmacist/competent person’s details (registration certificate, educational qualification, experience certificate)
- g) Applicant details (firm/company constitution, photos, ID/address proof, legal tenancy affidavit)
- h) Details of storage accommodation
- i) In case of change of premises or change in the constitution, enclose the following:
- *original license
- *documents related to change in the constitution like sale deed, reconstitution/dissolution deed,.
- *Amalgamation court along with board resolution and any other document order if it is company.
- *Purchase bills of A/C and refrigerator along with Working condition/Installation Certificate.
- *Any other document or particulars to verify the correctness of the particulars submitted by the applicant if required.
II) To apply under Mudhalvar Marundhagam scheme
- 1) B.Pharm / D.Pharm / consent letter of B.Pharm/ D.Pharm pharmacy license holder
- 2) Council registration
- 3) Pharmacy council
- 4) Drug license
- 5) Retail license
- 6) FSSAI Certificate
- 7) Ownership/ rental agreement documents
- 8) Property tax/ water tax/ EB Bill
- 9) GST
- 10) PAN card
- 11) Aadhar card
- 12) Bank passbook
- 13) Consent letter if rental
- 14) (Destitute widow / SC/ ST/ differently-abled certificates of applicants
